  • Care Coordination:  Collaborate closely with physicians, nurse practitioners, dietitians, and other team members to execute comprehensive care plans.
  • Documentation:  Accurately document patient interactions, updates, and care coordination notes in the electronic health record (EHR).
  • Technology Utilization:  Leverage telehealth platforms, technology-based patient engagement tools, and communication software to support effective remote patient management.

Qualifications

  • Bachelor’s degree in Health Education, Nutrition, Psychology, Public Health, or related field (preferred).
  • Certification in Health Coaching from a recognized program (strongly preferred).
  • Minimum of 1 year experience as a Health Coach, Patient Navigator, or similar role (required).
  • Experience working in obesity, weight management, lifestyle modification, or chronic care management (preferred).
  • Telehealth or virtual patient engagement experience (preferred).
  • Technical Skills: Proficiency with telehealth platforms, EHR systems, Google Workspace (Gmail, Docs, Sheets), and other digital communication tools.
  • Exceptional interpersonal, communication, and motivational interviewing skills.
  • Ability to manage multiple patient interactions efficiently and empathetically.
  • A reliable internet connection and a private, HIPAA-compliant workspace at home.

Schedule & Compensation

  • Hourly Rate:  Competitive, commensurate with experience.
  • Hours:  Flexible part-time schedule with potential transition to full-time based on patient volume and performance.

  • Benefits:
    • Possibility of pro-rated benefits for part-time or full benefits package for full-time (medical, dental, vision, etc.)
    • Continuing education stipend or professional development opportunities (varies by role classification)
    • Potential for growth within a fast-evolving telehealth organization
