MedAdventures is seeking a compassionate and motivated Physical Therapist (PT) to join a Continuing Care Retirement Community (CCRC) team in Springfield, OH . This is a great opportunity to work with older adults in a supportive, resident-focused setting that values continuity of care and quality outcomes.

You’ll be part of a collaborative rehab team providing therapy services within the CCRC campus, helping residents maintain or regain independence through personalized, goal-driven care.

Responsibilities:

• Evaluate and treat residents to improve function, mobility, and quality of life
• Develop individualized care plans and update as needed
• Collaborate with nurses, physicians, and the interdisciplinary team
• Educate residents and caregivers on home exercises and fall prevention
• Maintain thorough and timely documentation per facility standards

Qualifications:

• Active Ohio PT license (or eligibility to obtain)
• Prior experience in a SNF or CCRC setting preferred
• Strong communication and clinical reasoning skills
• A genuine interest in working with geriatric populations
• Ability to work effectively within an interdisciplinary team
